• Professional licenses and certifications for all medical practitioners, nurses, and healthcare professionals employed by the institution.

• Certificate of compliance with tax obligations issued by the Edo State Internal Revenue Service (EIRS) or relevant tax authority.
• Any other relevant documents as specified by the ministry during the registration process.

Upon receipt of the registration application and supporting documents, ministry officials will conduct a thorough inspection and evaluation of the private health institution to assess compliance with regulatory standards.
