• Business registration certificate issued by the Corporate Affairs Commission (CAC) or relevant regulatory authority.
• Professional licenses and certifications for all medical practitioners, nurses, and healthcare professionals employed by the institution.
• Facility assessment report detailing the infrastructure, equipment, and amenities available at the institution.
• Certificate of compliance with tax obligations issued by the Edo State Internal Revenue Service (EIRS) or relevant tax authority.
• Any other relevant documents as specified by the ministry during the registration process.
Upon receipt of the registration application and supporting documents, ministry officials will conduct a thorough inspection and evaluation of the private health institution to assess compliance with regulatory standards. The inspection may include a review of facilities, equipment, staffing levels, infection control practices, and adherence to professional ethics and guidelines.
Private health institutions that satisfy the eligibility criteria and demonstrate compliance with regulatory requirements will be granted approval for registration by the ministry. Upon approval, a registration certificate will be issued, authorizing the institution to operate and provide healthcare services within the state.
